Endocrowns

A single piece of ceramic for a tooth that has had root canal treatment — no post down the root, and no reducing the whole tooth for a crown.

An endocrown is a single piece of ceramic for a tooth that has had root canal treatment. Instead of anchoring a metal post down into the roots, it uses the hollow chamber the endodontics left behind, and sits as one solid block that takes the bite.

A treated molar has lost internal structure and is prone to fracture under load. What protects it is coverage of the biting surface; where the outer walls are still standing, that coverage does not have to come from a full crown.

What the walls still hold

Whether the molar keeps solid outer walls is what decides if an endocrown is an option at all, and it is checked before anything is prepared.

Made outside the mouth

The piece is produced against a precise record of the chamber and the remaining walls, so it seats as one block instead of being built up in layers.

Bonded, then checked in function

It is bonded to the remaining tooth so the two behave as a single unit, and the bite is verified in movement before it is finished.

When it fits

No post inside the root, and no reducing the whole tooth for a crown. But it is not for every case — it fits two in particular.

The walls are still there. The molar has to keep solid outer walls for the piece to seat and bond.

There is little vertical space. When the bite clearance is too small for a conventional crown, this is often what fits.

Why it is not offered more often

What makes it strong is what makes it hard to undo.

If the root canal fails later and the roots have to be re-treated, getting through a bonded endocrown is considerably harder than through a crown — a crown can be perforated or taken off; this cannot.

And if it ever has to be replaced, it comes out by drilling it away, because it is a single piece.

That is why it is offered for the cases that fit it, and not as a default.

Frequently asked

What patients ask before they book. If yours is not here, the fastest answer is a call.

15 to 20 years or more in cases that were well selected. Keeping the natural walls and skipping the post inside the root lowers the risk of a root fracture, which is the failure that ends in extraction.

Normal diet the same day — the bond reaches its strength almost immediately. Keep ice and hard candy off that side for the first 24 hours.

Because the post is drilled into the root, and that is where the risk sits: a root fracture around a post is the failure that ends in extraction. Where the outer walls are still standing, an endocrown gets the same coverage without entering the root at all.

Whether your treated molar needs a crown or an endocrown is a question about its walls, not about preference.

It is answered by looking at what the tooth has left after the endodontics, and it is written into the plan before anything is prepared.

If you have been told a treated molar needs a post and a crown, it is worth asking whether the outer walls are still sound. That single question changes the plan more often than patients expect.
