Dental Implant Repair & Rescue
An implant that hurts, moves or embarrasses you is not a lost cause until an implant specialist has examined it.
Implants are built to last, but the parts around them live a hard life: a crown works loose or chips, a retaining screw loosens, the gum around a post becomes inflamed, or an implant placed somewhere else never quite felt right. None of those is a reason to live with discomfort — and none of them automatically means losing the implant.
A three-dimensional CBCT scan shows the bone around the implant, its position and what is actually recoverable, and the answer you get is built on that image rather than on a guess.
Rescue cases here are assessed by Dr. Jorge Maytorena and Dr. Rafael Cortés Álvarez — a two-surgeon implant team, so the call on whether an implant can be saved carries more than one set of trained eyes.
Who assesses a rescue case
Dr. Jorge Maytorena is a leading surgeon on the dental team. He works closely with the DASS protocol (Digitally Assisted Soft tissue Sculpturing) — the digital workflow developed by Dr. Alessandro Pozzi and taught alongside Dr. Lorenzo Arcuri. This cutting-edge technique maps the bone and gum interface on screen before surgery, so the arch placed that same day is shaped to guide how the tissue heals; this allows a finished full-arch case to read as teeth, rather than as pink acrylic. His practice in Baja California focuses on this advanced implantology and complex oral rehabilitation, with an emphasis on precise clinical planning and a comprehensive approach developed during his specialized training at Instituto Alzaga. He completed his foundational studies as a Dental Surgeon at CEUS Universidad, and all implant surgeries are carried out jointly, ensuring that critical decisions always benefit from collaborative professional expertise (Cédula profesional 13741153).
Expertise on every implant case
Dr. Rafael Cortés Álvarez, Director of Makeover Dental Studio, is a pioneer of guided implant surgery in Baja: in 2006 he placed the region’s first All-on-6 case using NobelGuide 3D technology, and he has worked with Nobel Biocare for the nineteen uninterrupted years since. He graduated from the Universidad Autónoma de Baja California and holds a diploma from Loma Linda University. His work carries the imprint of two lineages: the direct mentorship of Professor Dr. Alan Kleiman, a pioneer in modern implant research, and the advanced protocols of Dr. Sascha Jovanovic’s continuing-education system, which he keeps refining alongside faculty from the world’s leading implant programs. That is what mentorship passes down, case by case — and it is the method behind the plan presented to you. Cédula profesional 5933630.
He was the first in the region to work with NobelGuide, at a point when computer-planned surgery was unfamiliar territory for most of the profession. Position, angle and depth are decided on the scan and transferred to your mouth with a custom guide.
How a rescue case is worked up
Imaging before any verdict
A CBCT scan in three dimensions shows the bone around the implant, the position of the post and the state of the components. No opinion is given before that image exists — on a rescue case, guessing is how implants get lost.
The fix matched to the failure
A loosened screw is retightened or replaced. A chipped or failing crown is remade on a digital 3Shape workflow. Inflamed tissue around a stable implant is treated so the bone stops paying for it. When the implant itself is sound, the implant stays.
An honest path when it is not
Some implants cannot be saved, and you will be told so from the imaging, not after months of patching. Removal, healing and, where needed, regeneration come first — then a replanned restoration on Nobel Biocare, Straumann or Hiossen systems, with the reasoning in front of you.
My implant was placed somewhere else. Will you see me?
Yes — a large part of rescue work is exactly that, and you will not sit through a lecture about where you went first. Records and the implant brand help if you have them, but they are not required: the CBCT scan is taken here and the work-up starts from what your anatomy shows. If the honest answer is that the implant is fine and only the crown needs attention, that is the answer you will get.
Is a loose or painful implant an emergency?
It is not something to sit on. Movement usually comes from the crown or the screw rather than the implant, and caught there it is a repair, not a loss. But inflammation around an implant does not settle on its own, and bone that is being lost does not come back by waiting. A prompt evaluation is what keeps a small problem small.

Frequently asked
What patients ask before they book. If yours is not here, the fastest answer is a call.
- Your case planned in 3D before treatment begins
- A treatment plan written in English
- A direct line to the clinic once you are home
Usually not. Most movement comes from the retaining screw or the crown itself, and both can be corrected without touching the implant. The evaluation and the imaging tell the difference before anything is decided.
Inflammation of the tissue around an implant that, left alone, starts to cost you the bone holding it. Caught early it is treatable and the implant is kept. This is the main reason a painful or bleeding implant should be looked at promptly rather than watched.
Yes. The brand and platform are identified from your records or from the imaging, and we work with Nobel Biocare, Straumann and Hiossen systems. Using components that genuinely match the implant is a large part of why a repair holds.
Rarely. Screws, abutments and crowns are replaceable parts. The implant itself is only removed when the imaging shows it cannot be kept — and in that case you hear the options and the reasoning before anything is done.
Whatever you have: the implant brand card, previous records or imaging. If you have nothing, that is fine — the CBCT scan is taken here and the evaluation starts from zero anyway.
An implant problem does not fix itself — but most of them can be fixed.
