All-on-4 Dental Implants
All-on-4 is not the answer to a missing tooth. It is the answer to an arch that is failing — teeth already lost, or teeth that cannot be saved and are on their way out. If that is where you are, the real choice in front of you is between something removable and something fixed.
The technique places two implants at the front of the jaw, where bone is densest, and two angled toward the back. Tilting those posterior implants engages more bone along their length and moves the support further back without entering the sinus or the nerve canal.
That angulation is the reason many patients who were told they needed a bone graft turn out not to need one. It works around the anatomy instead of rebuilding it first, which is also why the plan is decided from imaging rather than from a brochure.
Who performs an All-on-4
Implant surgery here is performed by Dr. Rafael Cortés Álvarez, who placed the first computer-guided implant case in the region in 2006 and has worked with Nobel Biocare for nineteen uninterrupted years. He graduated from the Universidad Autónoma de Baja California, certified with a diploma from Loma Linda University, and trained under Dr. Alan Kleiman. Cédula profesional 5933630.
All-on-4 is a full-arch case decided by bone volume and angulation, not by a package name. The technical foundations of what the field now calls All-on-4 are the same ones he was building in 2006, and the position of every implant is settled on the scan before anything is opened.
Two implant surgeons, one All-on-4 standard
Dr. Jorge Maytorena also performs All-on-4 surgery at Makeover Dental Studio, working alongside Dr. Rafael Cortés Álvarez. He works closely with the DASS protocol (Digitally Assisted Soft tissue Sculpturing) — the digital workflow developed by Dr. Alessandro Pozzi and taught alongside Dr. Lorenzo Arcuri. This cutting-edge technique maps the bone and gum interface on screen before surgery, so the arch placed that same day is shaped to guide how the tissue heals; this allows a finished full-arch case to read as teeth, rather than as pink acrylic. His practice in Baja California focuses on this advanced implantology and complex oral rehabilitation, with an emphasis on precise clinical planning and a comprehensive approach developed during his specialized training at Instituto Alzaga. He completed his foundational studies as a Dental Surgeon at CEUS Universidad, and all implant surgeries are carried out jointly, ensuring that critical decisions always benefit from collaborative professional expertise (Cédula profesional 13741153).
How a full arch case moves
Imaging before opinions
A three-dimensional scan maps bone volume, sinus position and nerve path. The number of implants and their angles are decided there, before anyone is in the chair.
Placement in one session
Teeth that cannot be saved are removed and the implants placed in the same surgical appointment, so the surgical phase runs once instead of across months.
Provisional, then definitive
A fixed provisional bridge is generally fitted while the implants integrate. The definitive bridge is made once integration is confirmed, not before.
Am I a candidate for All-on-4?
It depends on bone volume, the condition of the remaining teeth and how your jaw closes. A scan answers it directly, and when four implants will not carry your arch you will be told that rather than talked into it.
Will I leave without teeth?
In most cases a fixed provisional bridge is placed the same day as surgery. It is a provisional by design: it carries you through healing while the definitive bridge is made.
What it costs
Prices in USD. Every plan is confirmed after imaging and clinical evaluation; the written estimate you receive before treatment is the one that applies.
Before & After
Full-arch cases are documented from the first scan onward: photographs and imaging before treatment, the provisional in place, and the definitive bridge once it is fitted. What the comparison shows is not only the smile — it is the bite and the lip support that a collapsing arch had been quietly changing.
What patients say
“Thank you so much for saving the day. I don’t remember the last time I had such a painful toothache. My wisdom teeth were definitely not helping. Thank you for taking me in last second…”
“From the moment I first reached out by email, my experience with Makeover Dental Studio has exceeded every expectation. The clinic itself is state-of-the-art, but what truly sets it apart is the incredible team behind it.”
“I went in there as a walk in and came out a happy customer. Great service and beautiful facility.”
“Very pleased and impressed with the professional service and attention given at this clinic. All of the staff is very attentive. Dr. Erika answered all my questions and concerns, she explained in detail the whole process of my braces…”

Frequently asked
What patients ask before they book. If yours is not here, the fastest answer is a call.
- Your case planned in 3D before treatment begins
- A treatment plan written in English
- A direct line to the clinic once you are home
A denture rests on gum tissue and moves under load. A bridge on implants is anchored to the jaw and transmits force through bone, which is why the two behave differently over the years, in comfort and in what happens to the bone underneath.
The treatment has two clinical phases: surgery with the provisional, and the definitive bridge after integration. Cross-border cases are planned around that split, with reviews handled remotely in between, and the full schedule is written out before the first appointment.
Implants are designed to stay in service long term, and what determines that is hygiene, bite control and follow-up rather than time alone. The bridge on top is a restoration and may be renewed over the years.
Request your consultation
A consultation for a full-arch case is a diagnosis, not a sales appointment: imaging, a written plan, and the reasoning behind the number of implants. If your anatomy points to a different solution, that is what the plan will say.
