Insurance
Whether your plan reimburses treatment abroad is decided by your plan, not by the border. Documentation is what lets you find out.
The short answer
| Your plan says | What that means here | What to do |
|---|---|---|
| Reimburses out-of-network care anywhere | Treatment here is claimable | Ask for an itemized statement with procedure codes |
| Excludes care outside the United States | Treatment here is not reimbursable | Worth confirming in the policy wording, not by assumption |
| Silent or unclear | The wording settles it, not the border | Call your insurer before treatment and ask directly |
Makeover Dental Studio is out-of-network for every U.S. plan by definition, because it is outside the United States. That is not the same as nothing being payable.
How an out-of-network claim works
Treatment
You are treated and you pay the practice directly.
Itemized statement
Each procedure listed separately, with its code, date and tooth or area.
You file the claim
The statement goes to your insurer under your plan's out-of-network terms.
Reimbursement
Your insurer pays you directly, at whatever rate your policy sets.
What you get for the claim
- An itemized statement, not a receipt for a total
- Standard procedure codes for each item
- The date each procedure was performed
Also available on request
- The tooth or area involved, per procedure
- A clinical narrative where the insurer asks for one
- Radiographs supporting the treatment
Itemized documentation
Each procedure listed separately with its standard code, the date and the area treated. A single lump sum on a receipt is difficult for any insurer to process.
Predetermination for larger cases
For substantial treatment, submitting the plan to your insurer before starting produces an answer in writing rather than an assumption.
Your questions, answered in advance
Out-of-network percentage, exclusions for care outside the country, annual maximum. Four questions to your insurer settle most of it.
Before you assume either way
Patients frequently assume their plan covers nothing abroad and never ask, and others assume it covers the same as at home and are surprised at the reimbursement. The policy wording settles it, and a call to your insurer before treatment is a short call with a specific answer.
- Ask about out-of-network coverage for care outside the U.S.
- Ask about your annual maximum and when it resets
- Ask whether a predetermination is worth submitting
Will my dental insurance pay for treatment in Mexico?
Some PPO plans reimburse out-of-network treatment regardless of where it was performed, and some exclude care received outside the United States explicitly. Both are common, and which applies to you is in your policy wording rather than in anything a dental practice can tell you. The call to make is to your insurer, and the questions are specific: does the plan reimburse out-of-network care, is there an exclusion for treatment outside the country, what percentage applies to the category your treatment falls into, and how much of your annual maximum remains. Fifteen minutes on the phone replaces a great deal of guessing.
What documentation do I need to file a claim?
An itemized statement rather than a receipt for a total. Insurers process claims by procedure, so each one needs to appear separately with its standard procedure code, the date it was performed, the tooth or area involved and the amount paid. Supporting radiographs are sometimes requested for larger treatments. A single figure labeled as dental work is the most common reason a legitimate claim is delayed or rejected, and it is entirely avoidable.
Frequently asked
What is a predetermination and should I get one?
It is a submission of the proposed treatment to your insurer before it happens, so they state in writing what they will reimburse. For substantial treatment it is worth the delay, because it converts an assumption into a commitment.
I am crossing from San Diego. Does the practice bill my insurer directly?
That depends on the plan and it is worth confirming before treatment rather than after. In most out-of-network arrangements the patient pays the practice and files for reimbursement, using the itemized documentation provided.
Does the annual maximum change anything about how I plan treatment?
It can. Where treatment divides naturally into phases and your maximum resets in January, sequencing the phases across two plan years is sometimes worth discussing. It is a scheduling question rather than a clinical one, and it only applies where the delay is clinically harmless.
Documentation for your claim
If you intend to claim, say so before treatment begins rather than afterwards. What the paperwork needs to contain is easier to get right the first time than to reconstruct later.
